we have a LLBLGen 5.4 license, but when I go to My Account -> Downloads I am unable to find anything related to ORM Profiler download link.

The ORM Profiler is available for everyone with a valid active subscription or when you have purchased ORM Profiler in the past when it was a paid product. If your subscription has ran out, the ORM Profiler isn't available to you, sorry.

Ok, lets assume I will prolong my subscription for one year and download ORM profiler. Does it also mean that once license becomes inactive again (after one year) I cannot use it anymore?

That's right, as it's a free tool given to people with an active subscription.
