Attached is a quick & dirty port I made for a customer. It is meant to make it easier to migrate to v3.5/v4.x if you're on v2.x and you use these templates.

NOTE: It's highly recommended you refactor your code so you're no longer tied to these templates. We didn't write the original templates (they're a 3rd party contribution) nor will we fix bugs in them.
